ABSTRACT:
Wyckoff Heights Medical Center aims to provide integrated Behavioral Health services for patients accessing primary care services. The primary care ambulatory practices systematically screen for behavioral health and provide treatment or referral as clinically indicated. For all new patients, annual visits, and when clinically warranted, patients are given the PHQ-9 and GAD-7 screening as part of the practice intake process.
Improving the assessment of anxiety and depression severity is important in guiding treatment decision.
A goal was set to improve the behavioral health screening rates for anxiety and depression for all new patients and annual visits to the minimum threshold of 80 %.
